"Rolling Guitar" I believe it's called, built by Jay Ohrberg on a 1969 Cadillac Eldorado.

Very very well done Tuckeroo! I had it as "Elvis' Fantasy", built by designer with a surname Orberg. As my source isn't very reliable/exact I give You two points. And, btw, if You can give me an internet link by PM showing an identical picture, I'll replace my picture with an unmodified one.
