Well, it looks like a sort of hybrid. An Iso Rivolta, with a Grifo front end grafted on.

I have found some photos of this car, but not much information. It is an Iso Rivolta, modified by a Spanish coachbuilder called SERA, of Barcelona. The rear of the car was also modified, and three examples were built, but I haven't found any more details than that.
