Hmmmmm.  Is it a variant of the Bizzarrini Europa 1900?  The one that was based on the Opel chassis?

No, it's not that.

Some characteristics of this cars reveal a relationship with Bizzarini 5300.
I think it's a special 5300, but I really don't know who did this.

Yes, it has a relation to the 5300. Bizzarrini only built two different production cars, so, this connection was quite likely...
However it's indeed a special one. And it's the only ever built.

I think this car could be a prototipe for a new style of the 5300, which was never built.
Designer could be Giugiaro (Italdesign).

There was Bizzarrini 5300 Stille Italia Spyder prototype with pop-up headlights, so maybe this is  Bizzarrini 5300 Stille Italia Coupe?

My source reveals nothing about an external design studio, so I think it was an in-house design.

A crazy guess... one of the last unfinished Bizzarrini 5300 completed some years later with a 6-cylinder Fiat 2300 engine?
