In my pic there is Dick Powell in an Aerocycle, 1936, a go-kart the company was trying to promote. Buck Jones also rode an Aerocycle.
Now for the 3rd point, is there any other info, beside that?

Only that Buck Jones said to do 20 mph on his one-man-power, two-speed, front-wheel drive vehicle.

Nothing on who built it I'm affraid.
