It's indeed a Phänomen Krankenwagen, but not by Miesen.

My source says it's based on a Phänomen 25 which in fact is a Granit as far as I understand.

I found a picture of this ambulance. It is a Phänomen Granit from 1938 and was used by the municipal hospital in Berlinchen/Neumark, today Barlinek. Still no idea about the coachbuilder.
