This car (called 'Mylady Lunga') is a one-off built in only 2 exemplars (a two-off then?) by Felice Mottola.

From what I understand on an Italian blog, they made other cars as well, seemingly, this is a prototype or something of which just 2 made, but of others more have been made. Interestingly, I have never heard of Mottola?

It looks Fiat-based(?).  Does anyone know which model?

No, definitely not Fiat based. Only the rims are (or look like) from Fiat 126.
It has a 125cc scooter engine, I think a Minarelli.

I believe it's a 2 cylinder 2 stroke 250.
