This is not an easy puzzle, I think...
So: one point for the correct identification of make, model and year; and one point if you can tell me where, when and with whom it crashed so heavely.

0614MDTR was sold in December 1956 to William Helburn of New York and was raced by the new owner in the Nassau Speed week races, in 1957 it was sold to Bob Said, who had raced a 500 Mondial before, he raced it in the Nassau Speed week races too. In 1958 it was sold to James Place he raced it in Road America, Elkhart Lake and Meadowdale, in 1958, '59 '60. In 1959 a Chevy engine was installed. in November 1960 the car crashed in Elkhart Lake the driver was killed. Bruce Lavachek, the "King of Waterbeds" found the car but could not negotiate a deal with Terry Fox in 1977 and then the traces of 0614MDTR disappear.

Chassis #0610MDTR ?

58/may/05 - badly crashed by Fred Geitel with a truck when practicing for the Helsinki GP. Only left rear fender and the tip of the rear bodywork survived. Rest of the body fabricated by "Autokoriteollisuus Oy" in Helsinki in just 6 days. As sister car 0702MDTR was used as a model, the new body work combines elements from 500TRC with older style 500TR. Factory mechanic Aldo Bertani was sent over from Italy to inspect mechanical components which had survived the crash with no severe damage.

After a lot of attempts you got it... ;)
It's 0610MDTR, crashed at Helsinki on may1958 (I have 58/may/06...) with Curt Lincoln and Fred Geitel...
