Looks like a modified Chrysler Voyager. Is it some sort of concept car based on this?

It's from a Chrysler brand. And it's been modified from stock.

By 'Chrysler Voyager' I mean offcourse the platform (Chrysler Town & Country / Plymouth Voyager / Dodge Caravan ), but this car was only sold in Europa as Chrysler Voyager ...

I have it as Plymouth Concept Voyager from 1987 but I don't know anything about this car.

I hope I'm not being too pedantic.  The word "concept" is not capitalised in that piece so I don't think it is part of the name.  I think the writer is saying that Plymouth have produced a concept mini-van or concept van which they have named the "Grand Voyager".  Much as you might describe an early Smart as a "concept town car" or the first Corsair and Falcon as "concept compacts" .
