A truly beautiful car, OP.
The car us a Studebaker Sceptre/Wagonaire designed by Brooks Stevens and I think built by Sibona-Bessona. I have seen conflicting dates for this one, 1964 and 1965 so I hope I am close enough.
Edit: I just checked the Brooks Stevens Museum site and the photo caption for this car says Sept. 1963 so that is probably the most accurate answer
Edit 2: A little bird just told me the correct spelling for the builder is Sibona-Basano.

I think the puzzle pic (at least the wagon section) was an illustration, versus the finished car, but you've provided to information required for the point.
