Seems to be based on Porsche 914. Is it a homemade one-off or a commercially available kit ?

Correct.  It's a commercially available kit AFAIK so has a name.

I've found the photo posted in a forum by Aaron Burnham, the son of the car's creator. The conversion has been made in the early eighties.

Actually I don't think you need to - that's enough!
I thought it had a name but although he does seem to have made some kits from it I can't find a name either..
So I think we'll call it solved.
