What name was given to this race car, what is it based upon, and who built it?

It's known as the MG Bellevue Special.  It was built by Wilkie Wilkinson on an MG NA chassis originally for the 1935/36 racing seasons, and converted later to a single-seater using parts from an MG K-type (axles, brakes, wheels). 6 Amal carbs replaced by a supercharger.
