That is the "1928 Porter" used in the TV comedy "My Mother the Car" starring Jerry Van Dyke and (as the voice of the car) Ann Sothern.  The car was based on a 1924 Ford T hot rod built by Norm Grabowski and modified for the TV show (I would assume in 1964 or 1965) by Kaye Trapp.

I remember the series well!
As a kid I enjoyed it but I'm sure I would think it pretty dire today...!
Was Jerry van Dyke related to Dick then?
