Designed and built by Marian Kotlicki at the Elta plant in Łódź. I can't read Polish, but he seems to refer to the vehicle as 'SAM 1985' (I understand 'SAM' is simply an acronym for 'self-built car'). Here are photos of the version in this puzzle:
In the collection of the Lane Motor Museum in Nashville, Tennessee and identified as 'SAM-1985'

according to an article (mentioned in scans You've posted) it was supposed to be build by company named GEZA and of course it was designed by Marian Kotlicki
on the puzzle photo is shown mouldings for this car
