Designed and built by Marian Kotlicki at the Elta plant in Łódź. I can't read Polish, but he seems to refer to the vehicle as 'SAM 1985' (I understand 'SAM' is simply an acronym for 'self-built car'). Here are photos of the version in this puzzle:

I went to the research and development center at the Fiat factory in Bielsko-Biała and showed them my project. They were interested, but they set conditions that did not suit me. according to an article (mentioned in scans You've posted) it was supposed to be build by company named GEZA and of course it was designed by Marian Kotlicki
