1915 Eagle-Macomber, aka Eagle Light Car.

With a 5-cylinder Rotary (or Axial) engine.

Eagle-Macomber and 1915 are correct but not the model name.  I locked the puzzle for you sixtee5cuda

Most contemporary literature calls it the "Eagle Light Car".  I found a couple of images calling it the "Eagle Cycle Car".  Both of these names are generic, and most small cars of the 1910's had "Light Car" or "Cycle Car" attached.  Model names were uncommon at this time.

Looks like an Eagle Model 1-A, because the seats are not staggered.
