Is this an NSU-Fiat 500C? Or a simple Fiat 500C? Is this a factory-fitted trunk or home-made?

Looking at the shape of the hood I would say it is not even a 500 C as that one had a different side line. I think this is a private attempt to make an old Topolino look like a 500 C up front as well as add some luggage space at the back.

Take note of the boot lid hinges near the bumper  :D

I agree: the rear part, between the rear fenders and the boot lid, is clearly from a first generation Topolino.
An odd and interesting attempt to have it looking like a 500 C.
