I think it's more likely to be based on an Appia, as it seems to be very short and small!

According to a well-known book it has an Appia engine (but my italian is not 100%....). Here are 2 more pictures

According to one observer the car was built by Sabatino Paganelli for racing driver Luigi Bellucci and is based around a Fiat 1100 chassis with a Lancia Aprilia engine.
